The presence of an external object in the periapical tissue by creating a foreign body reaction can lead to endodontic failure. This article describes the treatment of an unusual impaction of three foreign bodies into the periapical tissues. Case report: Three foreign bodies in periapical space of an adolescent was accidentally detected. He had trauma history in the anterior teeth. Non-surgical and surgical treatment was performed. After two years follow up, clinical signs showed improvements and the bone was repaired. Conclusion: Foreign body reaction in the periapical tissue and the delay in its detection cause extensive destruction of the bony structure.
